Our Aged Health Checks Include
Medication Management Reviews (approx. 60 minutes)
This review involves a discussion of current medications to identify potential issues and support safe use. Where appropriate, a review may be arranged in collaboration with a pharmacist.
Health Check for people aged 75 and older (annual) (approx. 60–90 minutes)
This annual health assessment provides an opportunity to discuss health concerns with independent doctors and consider appropriate assessments and referrals based on individual needs.
Coordinated Veterans’ Care (CVC) Program (approx. 60–90 minutes)
The CVC Program supports eligible Gold Card holders with chronic conditions through coordinated care involving medical professionals. Our doctors may work with your GP and other providers to support ongoing care planning and follow-up, where appropriate.

- You can discuss appropriate health checks for conditions such as skin disorders, glaucoma, osteoporosis, bowel cancer, and diabetes with your doctor.
Healthy Lifestyle Checks
- The Healthy Lifestyle Checklist provides an opportunity to review lipid, glucose, and blood pressure measures and discuss results with your doctor.
Immunisations
- Immunisations such as influenza, pneumococcal, and shingles vaccines may be available for eligible patients aged 65 and over, and for those with certain chronic conditions, in accordance with clinical guidelines.
Advanced Care Planning
- Advanced care planning allows discussion of preferences for future care. Our doctors can discuss available options and referral pathways, where appropriate.
Palliative Care
- In collaboration with trained teams, our independent doctors and nurses support palliative care planning and coordination for patients with advanced illness.
Nursing Home Visits
- This service is available following prior discussion with a doctor and may be offered on an individualised basis.
